CLUB AND MEMBER CONTRIBUTIONS
The forms and documents in this section are to facilitate support for The Rotary Foundation by District clubs as well as individual Rotarians. All forms should be submitted to the District Foundation Administrator:

TRF Member Commitment Form
This form is for individual Rotarians to make an annual pledge to support The Rotary Foundation. It is part of Rotary's "Every Member, Every Year" campaign to encourage Rotarians to be regular supporters of the foundation. Clubs are encouraged to use this form to promote a total club commitment To the Rotary Foundation (such as a Centurian or Sustaining Member)
TRF Bank Authorisation Form
This form is for use by individual Rotarians to authorise their bank to make regular automatic contributions to the Rotary Foundation. The form allows the donor to specific the amount and the frequency of the automatic contributions as well as the period of time (for example: $20 per months for 2 years)
TRF Multi-Donor Form
This form is for use by the individual in the club (typically the Treasurer) responsible for reporting club and member contributions to The Rotary Foundation. This single form is for reporting both club contributions as well as individual contributions. It is the key form to insure that proper credit is given by for:
Club contributions and for Paul Harris Fellow credits
Club recognition for district Foundation support
Individual recognition and tax deduction credit
TRF Paul Harris Fellow Award application
This form is to be completed when applying for a Paul Harris Fellow Award. Clubs must have at least 1000 recognition points recorded (i.e. USD$1,000 contribution to The Rotary Foundation) before submitting an application. Clubs should allow four weeks for this application to be processed and shipping of the award certificate and pin.
